This product is for KAP Medical Equipment manufactured products only. This Service Evaluation Fee is required prior to your product being shipped to KAP Medical for evaluation. All service evaluation requests require a Return Authorization number, which will be provided to you within two business days of completing this Service Evaluation order. Instructions will also be provided at that time with details about where you will need to ship your product for evaluation. Note: Shipping cost to KAP Medical is the responsibility of the customer. Return Shipping is already included in your Service Evaluation Fee amount.
A Service Quote will be provided to you once your unit has been evaluated by KAP Medical for repair. If you Approve the Service Quote, the quoted cost will be applied to your approved payment method, and your unit will be returned to you upon completion of the repair.
If you instead choose to not have KAP Medical proceed with repairing your product, you will have two options to proceed with your unit:
A.) You can have the product shipped back to you (return shipping cost already included in the Service Evaluation Fee); or
B.) You can have KAP Medical dispose of the product (scrap fee already included in the Service Evaluation Fee)
Product Evaluation Lead Time
Evaluations are handled by KAP Medical and normally take 7 to 15 business days from the date your product is received.
General Service & Repair Lead Time
All service and repair work is performed by KAP Medical. The time to complete repair work is 4 to 6 weeks from the date you approve your Service Estimate Quote.

- $8.90 Flat Rate* for most items/orders under $75 unless otherwise stated
- 48 Contiguous United States only. Call for freight quote on orders shipping to Alaska or Hawaii.
- Additional shipping charges apply for items marked "Oversized" and/or "Overweight" and for destinations in Alaska, Hawaii and US territories. Express shipping is not available for freight items that weigh over 100 lbs. We do not ship outside of the United States.
- You have 30 calendar days to return an item from the date you received it, unless otherwise stipulated by the manufacturer.
- Items must be returned in New, Unused and Resalable Condition, and in its Original Manufacturing Packaging. Manufacturers will not grant returns if the original packaging has been discarded.
- There are certain products that are inherently Non-Returnable due to the nature of the product, sterile/hygienic concerns, or for custom-built reasons. These products include: Mattresses; Mattress Pads; Bath Lifts; Incomplete Case Packs; and other items that may have medical device return restrictions from the manufacturer.
- If a Case Pack item packaging has been opened, or if a Mattress or Bathroom Product has been put into service, it is NOT eligible for return.
